Patients can self-certify for 7 working days before a FIT note is required and FIT/sick notes can be back dated as required.
Please be advised that FIT notes cannot be issued over the telephone. Any requests for FIT/sick notes must be completed via PATCHs, and the sick note questionnaire should also be completed within PATCHs. Any patient that feels they cannot register with PATCHs should contact the surgery and we will help you.
